Blue Nautilus shell blister "pearl", facted heart shaped blue topaz. 4mm round stone set in the bail might be Aquamarine - almost clear but very faint blue color
the blue "pearl" is actually cut from the very center of a Nautilus shell; it is not actually a pearl, but because it looks like a mabe or blister pearl, this can be confusing.
